An opportunity has arisen for a Assessing and Reviewing Officer in an integrated CMHT which specialises in supporting, predominantly, older adults with complex mental health difficulties and their carers.
You will become part of a well-established and dynamic team that reviews stable community packages and placements in care homes and extra care provision. The person will hold reviews in local care homes and in service users homes.
Following a review the person will be expected to update care plans and risk assessments. The successful applicant will work closely with third sector providers, domiciliary care providers and other mental health teams.
You will be expected to maintain and develop the already positive relationships with local GPs, the Safeguarding and DOL's teams as well as private providers. The person who joins this cohesive and supportive team will receive regular training, monthly supervision and annual appraisal.
