The Extra care team based at Hawthorn court provide a step down service in partnership with Curo housing Association.
The team support people and operate to provide rehabilitation and support to adults - often frail and elderly, within a home environment to promote optimum levels of independence; to prevent hospital admission, to facilitate hospital discharge, maximise independent living in the community and assess requirements of appropriate on going care packages. This involves working closely with our District Nursing, Social Care and housing support Colleagues to provide person centred care.
The role of a Stepdown Support Worker is to support our service users to enable them to live as independently as possible within their own home. We support these individuals by providing personal care, basic health care and therapy such as mobility practice, supervising exercises and practice of techniques to help the individual to maximise independence as far as they are able.
We are a friendly team consisting of a Registered manager, care manager, shift leader and support workers. We work closely with our District Nursing, Social Care and housing Colleagues to deliver joined up care.
The successful candidate will be well supported and will work under the direction of a shift leader and or care manager, the Stepdown support worker and shift leader will help Service Users’ work towards their outcome-focussed re-ablement goals as set out in their support/treatment plan.
The working arrangements will include weekends and bank holidays on a rostered basis with split shift between the hours of 7am and 10pm.
